Home Hospice

If you or someone you love is at an advanced stage with a serious illness, you have no doubt heard of hospice care. But is hospice a place or a service? What Services does Home Hospice Care Provide?

Together with the patient, our Agapé Hospice team will establish a Plan of Care that addresses all the person’s needs. This multi-disciplinary team may include physicians, nurses, social workers, hospice aids, clergy, bereavement counselors and volunteers dedicated to helping you stay at home if that is your desire. Your care needs can be met with a few visits a week, daily care or on an on-call basis after normal business hours. Our home-care team members do not provide 24-hour care, but are available for emergencies 24 hours per day.

Home hospice care is not just available for the patient. Hospice is an inclusive service that also provides care for a patient’s family members. At Agapé Hospice, we understand the challenges of end-of-life care affect everyone connected to person who is ill. Spiritual and emotional support, counseling, and medical services are available to the patient’s caregivers and family. Patients receiving hospice care at home can expect the same level of care and services, including:

  • A Plan of Care developed by the patient’s attending physician and other trained hospice personnel

  • Comfort-oriented medical treatments
